HCFA (CMS-1500) Online Entry Form Enhancements: Claim-Level Adjustments and Rendering Provider Support
We’re introducing an enhancement to the HCFA (CMS-1500) Online Entry Form (OLE) in Service Center 2.0. This update adds support for claim-level adjustments on secondary claims and improves how Rendering Provider information is captured when submitting electronic claims.
Together, these enhancements provide greater flexibility, accuracy, and consistency when building and submitting claims.

What’s New in the Form?
Claim-Level Adjustments for Secondary Claims
The HCFA form now supports claim-level adjustments, aligning it with existing functionality available for UB-04 and ADA forms.
A Claim Level Adjustments section has been added to the bottom of the HCFA interface.
A blue informational callout guides you on where to enter Coordination of Benefits (COB) information.
Built-in validation ensures COB information is entered either at the Claim Level or Line Level, but not both.
Validation Behavior
If both the claim-level and line-level sections contain data, submission will be blocked and the informational callout will turn red.
If both sections are blank on a secondary claim, they will be highlighted as required.
Once you begin entering data in one section, the highlight on the other section clears automatically.
Rendering Provider Enhancements
Updates have been made to improve how Rendering Provider information is captured and submitted on electronic claims.
A new field has been added to Box 33 for entering a Claim-Level Rendering Provider NPI.
Conditional fields have been added at the line level to capture Rendering Provider details when the Rendering Provider differs from the claim-level provider.
Box 33: Claim-Level Rendering Provider
Used when a single provider renders services for the entire claim.
Box 24J: Line-Level Rendering Provider
Optional and only used when a different Rendering Provider exists for a specific service line.
Why This Change?
This enhancement aligns the HCFA OLE form with industry standards and supports more complex billing scenarios.
It ensures claims are built accurately when:
A single provider is rendering services for the entire claim, or
Multiple rendering providers are involved across different service lines.
What You Need to Do
When submitting HCFA claims through the Online Entry Form (OLE):
Enter the Claim-Level Rendering Provider NPI in Box 33 when applicable.
Use Box 24J only when a line-level Rendering Provider differs from the claim-level provider.
For secondary claims, enter Coordination of Benefits (COB) adjustment information at either the claim level or line level, but not both.
