How to Create Your First Intake Forms

If you are adding patient intake forms for the first time in Practice Mate/EHR, follow the steps below to get started.

Before We Begin

Make sure you have access to either Practice Mate and/or EHR 24/7. Intake forms are tied to your appointment scheduling, so you’ll need to access the Appointments tab to begin setup.

Steps to Create Your First Intake Forms

  1. Go to the Appointments Tab
  • From the Practice Mate/EHR dashboard, navigate to the Appointments tab.
  1. Click “Add” to Start a New Appointment
  • Click the Add button to open the Add New Appointment window. 
  1. Locate the Intake Forms Section 
  • At the bottom of the Add New Appointment window, look for the section labeled Intake Forms.
  1. Select “Click Here To Get Started” 
  • Select the Click Here to Get Started link to begin setting up your intake forms. This will guide you through the process of creating and customizing your forms.
