If you are adding patient intake forms for the first time in Practice Mate/EHR, follow the steps below to get started.
Before We Begin
Make sure you have access to either Practice Mate and/or EHR 24/7. Intake forms are tied to your appointment scheduling, so you’ll need to access the Appointments tab to begin setup.
Steps to Create Your First Intake Forms
- Go to the Appointments Tab
- From the Practice Mate/EHR dashboard, navigate to the Appointments tab.
- Click “Add” to Start a New Appointment
- Click the Add button to open the Add New Appointment window.
- Locate the Intake Forms Section
- At the bottom of the Add New Appointment window, look for the section labeled Intake Forms.
- Select “Click Here To Get Started”
- Select the Click Here to Get Started link to begin setting up your intake forms. This will guide you through the process of creating and customizing your forms.
