Rejection: TPO rejected claim/line because payer name is missing. (Use status code 21 and status code 125 with entity code )
What happened: The primary payer carrier code needs to be sent in the 'Secondary Claim Info' section where the payer ID usually goes.
Resolution: If that info is already?there, resubmit the claim?as we have updated our system on 7/3/12 to allow codes of 2 digits or more to be sent. If the payer ID is there, please refer to?the payer?for the correct carrier code.
