WHAT HAPPENED: Billing Provider Taxonomy code in box 33 is not correct.

RESOLUTION: Verify what the taxonomy code is they should be using for that provider. To find the taxonomy code, click HERE. On that page enter in NPI, click link below with the NPI, and scroll down the page. The taxonomy code will be listed.
