Rejection: Svc: (AZBlue) Resubmit with description of unclassified/unlisted procedure.

What happened: At least one of the CPT codes needs a line item note.

‍Resolution: Normally this has to do with J-codes. It is best to verify with the payer on the CPT codes billed and whether or not they need the line item notes.
