What happened: One of the diagnosis codes in Box 21 falls into the accident, injury, poisoning category. Whenever diagnosis code falls into that category, the accident date is required.

‍Resolution: Need to add the accident date in Box 15 (with qualifier 439) or the Additional Fields section (under box 25) and resend the claim.
