CPT cannot be used with modifier 50 (FE403)

Rejection: CPT 99213 thru 99215 and 99203 thru 99205 cannot be used with modifier 50 (FE403)

‍What happened: One of the above CPT codes were used in box 24d with the modifier of 50.

‍Resolution: Verify the codes in box 24d and update the claims as necessary.
