If you need to view, add, or manage ICD codes in EHR 24/7, use the Problem List. The Problem List displays all diagnoses documented in a patient chart, including both those added through progress notes (Encounters) and those entered manually.
Steps to Access the Problem List
Open the Patient Chart
- Navigate to the appropriate patient chart in EHR 24/7.
- Hover your mouse over Progress Notes in the top menu.
- Click on the Problem List under the Health Information section.
Steps to Add a New Diagnosis
- Add a New Problem
- Hover over the Add New button in the upper-right corner.
- Select Add ICD-9 or Add ICD-10.
- Select a Diagnosis Code
- When the Add Problem pop-up opens, choose a diagnosis code by:
- Hovering over Add Problem and selecting from:
- Or click the ICD-9 or ICD-10 hyperlink under the Type column to open the System Defined List.
- Enter Problem Details
- In the Problem Details section at the bottom of the pop-up, enter:
- Save the Diagnosis
- Click Update to save the diagnosis on the Problem List.
Viewing and Managing the Problem List
- Sort and Search Diagnoses
- From the top of the page, sort diagnoses by date, date range, or month.
- Use the Search For drop-down to search by:
- Diagnosis Code Description
- SNOMED Code or SNOMED Description
- Apply Display Filters
- Use the Display Filter section to filter by:
- Type (E = added in an encounter, M = added manually)
- Status (Active, Inactive, Resolved)
- Code Type (ICD-9 or ICD-10)
Editing or Updating Diagnoses
- Edit a Diagnosis
- Click Edit to the right of a listed diagnosis.
- Review or update details of a diagnosis, including status (Active, Inactive, or Resolved).
- Click Update to save changes.
- Delete a Diagnosis (Manually Added Only)
- Diagnoses added manually will display Added Manually beneath the provider’s name.
- These entries can be deleted using the option to the right of the diagnosis.
Additional Information
- Diagnoses added from a progress note will display a clickable Encounter ID, which opens the associated note.
- Patient education materials can be accessed by clicking the diagnosis description or the Patient Education button.
- Signed progress notes cannot be edited; diagnoses originating from those notes must be managed within the note if it is unsigned.
