How to Set Up SOAP Note Layouts in EHR 24/7

If you want to customize how patient encounters are documented in EHR 24/7, you can create and manage SOAP Note Layouts. Each user can have up to 10 custom layouts, and these are user-specific (they cannot be shared between accounts). .

Where to Access SOAP Note Layouts

To set up or edit SOAP Note Layouts, follow the steps below:

1. Log into EHR 24/7.

2. Click on Manage Office in the top menu.

3. Under User Section on the left-hand side, select SOAP Note Layout.

Steps to Create or Edit a SOAP Note Layout

1. Add a New Layout

• Click the Add New button at the top left of the page.

2. Enter Layout Details

• At the top of the page, enter a SOAP Note Layout Title.

• In the upper-right corner, select the default Encounter Type.

3. Customize Note Sections

• Use the checkboxes to select which sections to include in the layout.

• For options that can be expanded or collapsed as you are charting, you can optionally select 'Default to Expanded' here to ensure you do not have to expand the section every time you create or edit a note.  

• Custom fields appear as blank checkboxes with a text field. If selected, you must name the custom field in the text box.  

Note: Custom fields should be used carefully if you have multiple providers collaborating on a single note.  Each individual provider's custom note name may be different.  Carefully ensure your Providers are using the same custom fields for the same information if multiple providers or staff edit notes together.

• Some custom fields include an option to Edit Phrase List — this allows you to create a list of selectable phrases for that section.

• Fields marked with MU are relevant for Meaningful Use compliance.

• Under the Plan section, be sure to check the Medication box if the layout will be used for e-prescribing.

4. Save Your Layout

• Once your selections are complete, click Update to save the layout.

Editing Existing Layouts

You can return to the SOAP Note Layouts section at any time to adjust your layouts based on your documentation needs.
