How to Record PHI Disclosures in EHR 24/7

If you need to document an instance where Protected Health Information (PHI) was disclosed, EHR 24/7 provides a PHI Disclosure Log within each patient’s chart. Follow the steps below to locate, record, and review PHI disclosures.

Before We Begin

PHI Disclosures can only be recorded from within a patient’s chart. Make sure you have the correct patient selected before accessing the PHI Disclosure Log.

Steps to Add a PHI Disclosure

  1. Access the PHI Disclosure Log
  • Navigate to the patient’s chart.
  • Hover over the Others option in the top menu.
  • Under the Chart Information section of the drop-down, select PHI Disclosure Log.
  1. Start a New Disclosure Entry
  • On the PHI Disclosure Log page, click Add Disclosure of PHI.
  • A pop-up window will appear where you can enter all disclosure details.
  1. Enter Reporter and PHI Details
  • From the top of the pop-up, select the Reporter.
  • Choose the Provider from the drop-down menu.
  • Select the Type of PHI Disclosed. Available types include:
  • Chart Download
  • CMS Quality Measure
  • Export Clinical Exchange Information
  • Immunizations
  • Medical Records
  • Other
  • Patient List
  • Patient Visit Summary
  • Syndromes
  • Enter the Disclosure Date.
  1. Complete “Reported To” Information
  • In the Reported To section, enter the recipients:
  • Name
  • Address
  • City
  • State
  • Zip Code
  • Purpose of Disclosure
  1. Select the Patient PHI Disclosed
  • Under Patient PHI Disclosed, check all applicable items:
  • Name  
  • Date of Birth
  • Address
  • Phone
  • Medical Records
  • Other
  1. Save the Disclosure
  • Click Update to save the PHI disclosure record.

Viewing or Searching Existing PHI Disclosures

Once saved, PHI disclosures will appear on the main PHI Disclosure Log page. You can:

  • Search for who the disclosure was reported to, by provider, or by office.
  • Sort using a date range.
  • Filter using the same PHI types listed earlier.
