What is this Measure?

This Measure evaluates the percentage of children aged 1 to 20 who receive two fluoride varnish applications within the measurement period, as determined by a dentist.​

Step 2: Open a Progress Note for the Patient for a billable office visit

Step 3: Enter procedure code representing qualified encounter in the procedure section

Populating the Numerator:

Children who received two fluoride varnish applications on different days during the measurement period.

Populating the procedure code

Step 1: Select the procedure note SNOMED code

SNOMEDCT:

234723000

Step 2: Repeat for 2nd flouride treatment encounter

Important Note:

Two encounters are required for numerator.
