What is this Measure?

This measure focuses on Body Mass Index (BMI) screening and the implementation of a follow-up plan when necessary.

Populating the Denominator:

Are 18 years or older on the date of the encounter and have at least one billable office visit encounter during the measurement period.​

Populating the Numerator:

Have a documented BMI during the encounter or within the previous 12 months.

If the BMI is outside normal parameters, have a documented follow-up plan during the encounter or within the previous 12 months.​
