Rejection: 60111 InvalidData: Location: 2400 - SE00 Clm: If one Service Line (Loop 2400 SV1) contains a GY modifier, then all Service Lines must contain a GY modifier.
What happened: At least 1 line item contained the GY modifier, but it was not sent for all line items. This new edit went into effect on 10/12/13. Per BCBS, when 1 line contains the 'GY' modifer, all lines on that claim must contatin the 'GY' modifier.
Resolution: This is for all of BCBS.Steps to take1.Confirm with the payer if any of the line items need the GY modifiera. If the GY modifier is not needed then remove it from all linesb. If the GY modifier is needed on at least one or more lines, then add GY to every line or split the claim into 2 different claims (one containing the GY modifier lines, one without).?
