Rejection: Accident Date Required on Accidents and Workers Comp Claims (FE134)

What happened: Information was sent in box 10, an accident date was not sent (box 15 / Additional Fields) or claim was sent to a work comp payer and accident date was not sent.

Resolution: Add accident date in box 15 or the additional fields section (under box 25, see attached picture below).

NOTE: All work comp claims require accident dates to be sent.
