What happened:?One of the diagnosis codes in Box 21?falls into the ?accident, injury, poisoning? category. Whenever?diagnosis code falls into that category, the accident date is required.
Resolution: Need to add the accident date in Box 15 (with qualifier 439) or the?Additional Fields?section (under box 25, see attached picture below) and resend the claim.
