Rejection: 01 - INVALID MBR DOB
What happened: Member's Date of Birth (DOB) on the claim does not match the date of birth on file with the insurance payer.
Resolution: Verify the date of birth with the insurance payer, and update box 3 on the HCFA claim form. IMPORTANT: If the patient and the insured are the same (you have relation set to "self" in box 6) then make sure to update the Insured DOB as well in box 11a.
