Rejection: Leave Date Of Service Blank for PreDetermination Claims (2068)

What happened: Box 1 was marked as Request for Predetermination/Preauthorization so a date of service cannot be sent.

Resolution: Need to either update box 1 or remove the date of service from the claim and resubmit.
