Rejection: Missing/Invalid Other Insured Name (HCFA box 9, UB box 58) (FE338)

What happened: Box 9 (HCFA), box 58 (UB04) does not have the other insured's name.

Resolution: When box 11d is marked 'Yes' a name is required in box 9 (HCFA), box 58 (UB04). Update the claim as necessary.
