Rejection: Policy number not on file

What happened: Based on the patient information the claim, the patient ID is not correct.

Resolution: Double check the patient's insured ID card and then call payer if needed to verify information. If this has been verified, please obtain proof of the patient's eligibility from the payer and contact Customer Service via live chat, email ( or phone 360-975-7000 option 1.
