Rejection: Acknowledgement Returned as unprocessable claim-The claim encounter has been rejected and has not been entered into the adjudication system. Subscriber Claim submitted to incorrect payer.

What happened: Claim was sent to wrong payer.

Resolution: Contact the phone number on the back of the patients ID card and verify which payer should be receiving the claim.
