Rejection: PAYER, GROUP AND EMPLOYER FIELD DATA IS BLANK OR INVALID. AT LEAST ONE OF THESE FIELDS IS REQUIRED.

What happened: One of the above items was not sent.

Resolution: Verify if the insured ID card has a group number or employer name on it and add it to the claim. Contact payer if there are any questions.
