Rejection: ~Acknowledgement/Rejected for Invalid Information | Information submitted inconsistent with billing guidelines. Note: At least one other status code is required to identify the inconsistent information. | Unit or Basis for Measurement Code

What happened: A valid anesthesia CPT code was sent, but the modifier is not anesthesia modifier.

Resolution: The only valid modifiers to use on an anesthesia claim are: AA, AD, QK, QS, QX, QY or QZ.
