Kaiser 94135 - Missing or invalid information

Rejection: Acknowledgement Rejected for Invalid Information - The claim encounter has invalid information as specified in the Status details and has been rejected. Missing or invalid information.

What happened: This is an unspecific error message.  An additional message is generally sent to give further detail to what the issue is.

Resolution: Refer to the other message(s) on the claim.  There may be cases where this is the only message listed.  Below are known payers and resolutions when this is the only message listed.  If the payer your claim was sent is not in this list, please contact our customer service department via chat (below), phone (360-975-7000 opt 1) or email ( for further research.

Kaiser Permanente - North (94135) Providers should be calling NCAL instead of the clearinghouse. Please note, for CA, due to Department of Manage Care, CA payers must send further explanation (letters) to the providers and members explaining how that claim was processed/denied. In the letter it contains Kaiser NCAL numbers to call.
