Rejection: ~Acknowledgement/Returned as unprocessable claim | Entity not found.

What happened: An entity on the claim (provider, patient, etc.) is incorrect.

Resolution: Unless the payer sent to is specified below, please contact our customer service department (360-975-7000 opt 1) so we can pull up a copy of the payer report to see which entity is incorrect.

Date Payer Issue and Resolution
All Dates Aetna 60054 This is referring to the provider.  Need to verify how the provider is set up at the payers end: Verify if set up as a group or individual, the NPI(s) and Tax ID (see attached picture below).
