Rejection: Invalid Clia Number (FE405)

What happened: The CLIA number in the Additional Fields section does not fit the correct format CLIA numbers.

Resolution: CLIA numbers must start with 2 numbers, the letter D, then 7 numbers. Need to verify the CLIA and update the claim.
