Rejection: Billing NPI required when legacy Billing ID does not exist. (RC133)

What happened: Neither the NPI or the Billing ID were sent in box 33a or 33b.

Resolution: Need to add the NPI or the Billing ID in box 33a or 33b and update the claim as necessary.
